Indemnity Agreement
Lakewood International Academy (LIA)
The School and the Lakewood International Academy Board of Directors undertake to implement reasonable and generally acceptable measures to ensure the safety and well-being of all learners, teachers, and visitors to the School. However, due to the inherent nature of school activities, the School and the Lakewood International Academy Board of Directors do not accept any responsibility for accidents that may occur in the classroom, on the school grounds, or on the sports fields.
Acknowledgment of Risks
As a parent/legal guardian, I understand and accept the risks involved in my child’s participation in school activities and excursions. I acknowledge that while the School will take appropriate measures to ensure safety, it cannot eliminate all risks.

as the parent/legal guardian of the learner named below, who is enrolled and accepted by the School, hereby indemnify the School and the Lakewood International Academy Board of Directors (Registration Number 2023/108887/08) from any and all claims for losses or damages that may arise from any incidents involving my child during school activities, excursions, or while utilizing school transport.
Authorization for Excursions and Transport
I authorize my child, [Learner Name], to participate in all school excursions and activities arranged by their class or group during school hours. I also consent to my child using the transport provided by the School for these excursions. I understand that I indemnify the School and the Lakewood International Academy Board of Directors against any damages or losses I may incur as a result of such participation and voluntarily accept the associated risks.
Bus Service Acknowledgment
In the event that my child utilizes the bus service to and from the School, I acknowledge that this service is operated by an independent contractor. I understand that neither the School nor the Lakewood International Academy Board of Directors is responsible for any incidents that may occur during transit. I recognize that the Lakewood International Academy Board of Directors has established conditions for the bus service to ensure compliance with safety regulations and that drivers must have a proven and unblemished record.
